All Vital Records can be obtained through the City, Town or Village Clerk’s Office in which the birth or death took place. The requesting individual must present identification.

Certified copies of Birth or Death Records with an official stamp and raised seal are provided for
$10.00 per copy

Those who have the ability to request a Death Certificate:

  • The named individual
  • An immediate family member or first degree relative (husband, wife, mother, father, child) of the named individual
    If you are not an immediate family member, you must provide a signed and notarized letter from one of the immediate family members giving you permission to receive the document
  • Attorney for the named individual or estate
  • Executor/Executrix of the individual or estate (death certificate)
  • or have Power of Attorney for the named individual

Those who have the ability to request a Birth Certificate:

  • The person born in Cheektowaga named on the birth certificate who is 18 years of age or older
  • A parent of the person named on the birth certificate. The requesting parent’s name must be show on the birth certificate
  • A spouse, child or lawful representative who presents a court order

To Request by Mail:

A letter can be addressed to the Town Clerk’s office (Registrar of Vital Statistics) requesting a certified copy of a Certificate of Birth or Death to Cheektowaga Town Clerk – Vital Records, 3301 Broadway Street – Room 101, Cheektowaga, NY 14227

  • Vital Statistics Request Form or letter with details
  • Proof of identity and relation to the individual(s)
  • Check for the number of copies being requested

GENEALOGICAL SEARCH SERVICES
Search Fee: $10.00 per name, per enactment of Ch. 413 of the Laws of 1991
Genealogical Copy: $1.00
