Sample Insurance Certificate(link)

Any person, firm or corporation that will have a vehicle plowing within the Town of Cheektowaga, outside the Village of Depew and Sloan, MUST have a permit issued by the Town Clerk’s office. The Owner of the vehicle is required to complete the proper form.

Permit fee is $25 per vehicle and you may list up to four (4) vehicles on each permit form (you can make copies)
Certificate of Insurance with evidence of liability insurance, stating that a Notice of Cancellation, Non-Renewal or Reduction of Insurance shall be given to the Town of Cheektowaga at least thirthy (30) days prior to such cancellation – (see Required Insurance (link))
- Certificate of Insurance copies may be faxed to our office (716)686-3515
- Verify all fields are complete and “Certificate of Holder” is assigned to Town of Cheektowaga per sample
Permit is valid from October 1 thru April 30 of the following year and Non-Transferable
Owner’s physical address must be on application (P.O. Box is not acceptable)
